In Blox Fruits, Terror Eyes are a legendary material which can be used to craft the Terror Jaw, Monster Magnet, and Mythical Scroll. The only way to currently obtain Terror Eyes is as a drop after defeating the Terrorshark.
How to Defeat the Terrorshark
First, you'll need to head to the Third Sea, and into a Sea Danger Level between 2-6. The greater the Sea Danger Level, the more sharks will spawn, and the greater damage they'll be able to deal.
The boss bar below (found at the top of your screen in-game) will indicate that a Terrorshark has spawned nearby:
You'll then need to look for the Terrorshark nearby, which look like this:
Terror Eyes Drop Rate
After defeating the Terrorshark, you'll have the chance to receive 1 set of Terror Eyes as a drop, among various other rare items. Here are the possible drops, including their drop rates:

Terror Eyes have an estimated 10% drop chance based on player experiences and testing, though no official confirmation exists. Nevertheless, expect to find one piece every 10 or so Terrorshark kills.
There are rumors that its drop chance may increase with higher Sea Danger Level, but this is inaccurate. Higher Sea Danger only increases the spawn rate of Terrorsharks, giving you more opportunities to farm it.
